KINDERGARTEN REGISTRATION NOTICE

Online Forms

Registration for kindergarten and first grade for the school year 2008-2009 will be announced early in April. To Register for Kindergarten, a child must be 5 by September 30, 2008. To register for first grade, a child must be 6 by September 30, 2008.

To register a child for school the parent or guardian must:

  1. Bring two proofs of residence.
  2. Bring the child's birth certificate.
  3. Bring the child's immunization record.

The following immunizations are required prior to school entry:

  • D.P.T. (Diphtheria, Whooping Cough, Tetanus) (minimum 4 needed - including 1 after age 4)
  • Polio Vaccine (minimum 3 or 4 needed - including 1 after age 4)
  • Measles vaccine – 2 doses prior to entry into kindergarten through 5th grade, and grades 7 –12.
  • Mumps, and Rubella (German Measles) Vaccine (1 dose given after 12 months of age)
  • Hepatitis B - 3 doses for children born on or after January 1, 1993.
  • Varicella (Chicken Pox) Vaccine - For entry into Kindergarten through 4th grade and 6th through 10th grade (unless child has already had chicken pox)
  • Middle School and High school students will be required to provide documentation that the tetanus/diphtheria booster has been received within 10 years.                    

In addition, the child must have a health examination by a physician before the start of school or a scheduled appointment for one. Children not having proper immunizations or a physical will be excluded from school in September. 

Any requests for immunizations waiver because of medical or religious reasons should be received in writing. Forms for immunization exemption and physical examination are available at the Principal’s/Nurse’s office. Completed forms should be returned to the school prior to opening day in the fall. School nurses will be available to answer any questions during registration.

The Manchester Health Department offers immunizations at no charge at regularly scheduled clinics for those unable to obtain them from their health care providers.  If you should have questions regarding immunizations, please call the Manchester Health Department at 624-6466.
