Parent/Guardian of 6th and 8th grade students-

Manchester School District and
Manchester Health Department have implemented a new policy that requires
all students entering 6th and 9th grades have to have a physical
examination by a licensed medical provider.

This new policy is in effect September
2007. Students will be excluded from school if they fail to provide
documentation of a completed physical examination within one year prior to
entry into 6th and 9th grades up to a six-month period after starting 6th
and 9th grades (March 1,2008). This policy will provide an 18-month window
of time to provide the required documentation for 6th and 9th grade
students.

Please provide your school nurse with the physical exams for
6th and 9th grade entrance as soon as available.

Any medications taken at school require a written physician's order. This includes any over the counter medications - per school department and health department policy.
